Welcome to the website of Geise Dental ("Geise Dental," "we," "us," or "our"), serving patients at our Huntsville, Madison, and Clift Farm locations in Alabama. These Terms of Service ("Terms") govern your use of www.geise-dental.com (the "Site") and any forms, communications, and services made available through it. By accessing or using the Site, you agree to these Terms. If you do not agree, please do not use the Site.

1. Use of the Site

The Site is provided for general information about our practice, our providers, and the services we offer, and to help you contact us, request an appointment, and access patient forms. You agree to use the Site only for lawful purposes and in a way that does not infringe the rights of, or restrict or inhibit the use of the Site by, anyone else. You may not attempt to gain unauthorized access to any part of the Site, interfere with its operation, or use any automated means to collect information from it.

2. No Medical or Dental Advice; No Provider-Patient Relationship

Content on the Site, including descriptions of procedures, treatments, and oral health topics, is provided for educational and informational purposes only. It is not dental or medical advice, is not a substitute for a consultation with a licensed dentist, and does not create a provider-patient relationship. Every patient is different; treatment recommendations are made only after an in-person examination. If you have a dental emergency, call our office or, if it is a medical emergency, call 911.

3. Appointment Requests and Online Forms

Submitting a contact or appointment request through the Site does not guarantee an appointment. Appointments are confirmed only when a member of our team contacts you. Please do not send detailed health information, insurance numbers, or payment information through the Site's general contact form. Patient intake forms available for download on the Site are provided for your convenience; completed forms should be returned to the office directly as instructed.

4. Communications From Geise Dental

By providing your phone number or email address through the Site, over the phone, or in our office, you agree that Geise Dental and the service providers acting on our behalf may contact you regarding your care and our services, including appointment scheduling, reminders, confirmations, follow-up care, recall notices, billing, patient satisfaction requests, and practice updates. We may contact you by phone, email, or text message (SMS/MMS), including through automated systems.

5. Text Messaging (SMS/MMS) Terms

Program description. When you opt in, Geise Dental will send you text messages related to your care at our practice, such as appointment reminders and confirmations, scheduling, follow-up instructions, recall notices, billing notifications, two-way conversational messages with our team, and occasional practice announcements.

Consent. By providing your mobile number and opting in, you consent to receive these text messages from Geise Dental at the number provided. Consent is not a condition of receiving care or making a purchase. Message frequency varies depending on your appointments and communications with our office.

Message and data rates may apply. Your mobile carrier's standard messaging and data rates apply to messages you send and receive.

Opt out. You can cancel text messages at any time by replying STOP to any message. After you send STOP, we will send one final message confirming that you have been unsubscribed, and you will no longer receive texts from us unless you opt in again. You can also ask our office to stop texting you.

Privacy. Your mobile phone number and text messaging opt-in information will not be shared with or sold to third parties or affiliates for their marketing purposes. Information is shared only with the vendors that send messages on our behalf, and only for that purpose. See our Privacy Policy for details.

Carriers. Carriers are not liable for delayed or undelivered messages. Message delivery is subject to effective transmission from your network operator.

6. Privacy

Your use of the Site is also governed by our Privacy Policy, which explains what information we collect through the Site and how we use it. Protected health information is handled in accordance with our Notice of Privacy Practices, available on our Patient Forms page and in our offices.

7. Intellectual Property

Unless otherwise noted, all content on the Site, including text, graphics, logos, photographs, and page design, is the property of Geise Dental or its licensors and is protected by copyright and trademark laws. You may view, download, and print pages from the Site for your personal, non-commercial use only. You may not reproduce, distribute, modify, or create derivative works from Site content without our prior written permission.

8. Testimonials and Before-and-After Images

Patient testimonials and photographs on the Site reflect the experience of the individuals shown and are used with their permission. Results vary from patient to patient, and testimonials are not a guarantee of any particular outcome.

9. Third-Party Links and Services

The Site may contain links to third-party websites, maps, forms, payment portals, or scheduling tools that we do not own or control. We provide these links for your convenience only and are not responsible for the content, privacy practices, or availability of those third-party sites and services. Your use of them is subject to their own terms and policies.

10. Disclaimer of Warranties

The Site and its content are provided "as is" and "as available" without warranties of any kind, express or implied, including warranties of accuracy, completeness, merchantability, fitness for a particular purpose, or non-infringement. We do not warrant that the Site will be uninterrupted, error-free, or free of viruses or other harmful components. Information on the Site, including office hours, providers, services, and pricing, may change without notice.

11. Limitation of Liability

To the fullest extent permitted by law, Geise Dental, its owners, providers, employees, and agents will not be liable for any indirect, incidental, special, consequential, or punitive damages, or for any loss of data or goodwill, arising out of or related to your use of or inability to use the Site, even if we have been advised of the possibility of such damages. Nothing in these Terms limits any liability that cannot be limited under applicable law, including with respect to dental care actually provided by our licensed providers.

You agree to indemnify and hold harmless Geise Dental and its owners, providers, employees, and agents from any claims, losses, or expenses, including reasonable attorneys' fees, arising out of your violation of these Terms or your misuse of the Site.

13. Accessibility

We are committed to making our Site usable by as many people as possible. If you have difficulty accessing any part of the Site, please contact us using the information below and we will work with you to provide the information or service you need in another form.

14. Governing Law

These Terms are governed by the laws of the State of Alabama, without regard to its conflict-of-law rules. Any dispute arising out of these Terms or your use of the Site will be brought in the state or federal courts located in Madison County, Alabama, and you consent to the jurisdiction of those courts.

15. Changes to These Terms

We may update these Terms from time to time. The effective date at the top of this page shows when they were last revised. Your continued use of the Site after changes are posted means you accept the revised Terms.
